As school nurses, we are responsible for ensuring compliance with regulations set forth by the Massachusetts Department of Public Health. This includes maintaining up-to-date medical records and ensuring vaccine compliance for students. If your student has a physical exam throughout the school year, please submit their up-to-date physical form after their appointment. You may send the forms in with your student or email them to me at: kaitlyn.reynolds@watertown.k12.ma.us. Below are the specific vaccine requirements for preschool and preK students.
