A Note from the School Nurse

Hello Early Steps Families!

As school nurses, we are responsible for ensuring compliance with regulations set forth by the Massachusetts Department of Public Health. This includes maintaining up-to-date medical records and ensuring vaccine compliance for students. If your student has a physical exam throughout the school year, please submit their up-to-date physical form after their appointment. You may send the forms in with your student or email them to me at: kaitlyn.reynolds@watertown.k12.ma.us. Below are the specific vaccine requirements for preschool and preK students. I will be reaching out to families in the coming weeks if they have a student(s) not in compliance with state requirements. Please do not hesitate to reach out with any questions. 
 

Preschool and PreK Vaccine Requirements

Hib: 1–4 doses*
DTap: 4 doses
Polio: 3 doses
Hepatitis B: 3 doses or laboratory evidence of immunity
MMR: 1 dose** or laboratory evidence of immunity
Varicella: 1 dose**, a reliable history of chicken pox, or laboratory evidence of immunity
Lead testing and results


*Number of doses is determined by vaccine product and age the series begins
**Must be given on or after 1st birthday
